Requested Personal Information

The following is the requested information we need to verify your account and the different options you have to gain access to the Calgary Community-Net computer system.

   - User ID (account login name) of the account you want to verify
   - Your full name
   - Current address and postal code
   - Current telephone number

       We request a photocopy of either a:

   - driver's license
   - passport

       You can use your personalized cheque as identification if:


   - your personalized cheque is payable to the Calgary Community-Net
     Association.

     (When you send in your cheque, please make sure that your phone
     number and your User Id (account logon name) is written on the
     cheque)

 NOTE:
         All personal information that we receive will be kept for
         the Calgary Community-Net's purpose only. None of your personal
         information will be given or sold to any outside agencies.
